Duke Health Community Care (DHCC) offers home care services through four programs: Duke Community Infusion Services, Duke Community Home Health Services, Duke Community Hospice Services, and Duke Community Bereavement Services.

Duke Health Community Care draws on more than 20 years experience in delivering quality home care in central North Carolina and beyond.

Our Programs

Our 200 employees serve patients throughout North Carolina, Virginia, and South Carolina. In particular, we provide:

  • Home Health Coverage Area: We serve patients within a 60-mile radius of Durham including parts or all of Alamance, Chatham, Durham, Granville, Franklin, Orange, Person, Vance, and Wake.
  • Hospice Coverage Area: We serve patients in all or part of the following counties, Alamance, Chatham, Durham, Granville, Franklin, Orange, Person, Vance, and Wake.
  • Infusion Coverage Area: We serve patients in North Carolina, South Carolina and Virginia. Pharmacy services provided throughout the United States.

Duke Community Infusion Services

We provide compassionate, quality home infusion care using a multidisciplinary approach, and the most effective and efficient utilization of resources to achieve the best patient outcomes. DCIS serves patients throughout North Carolina, South Carolina, and Virginia.

Duke Community Home Health Services

We provide certified home health services for adult and pediatric patients needing nursing, rehabilitation and home health aides.

Duke Community Hospice Services

We provide hospice care in private homes, long term care facilities, and a six-bed inpatient facility in Hillsborough.

Duke Community Bereavement Services

We offer bereavement services to anyone in our community who has suffered a loss due to death or who requires critical incident debriefing.
